advance directive, is pleased to offer multiple language translations
developed with support from the United Health Foundation. Five
Wishes has become the most popular advance directive in America
because it is easy to use and understand, and it addresses the
personal, emotional, and spiritual issues that people say matter
most to them during times of serious illness. Thanks to new language
translations, Five Wishes can now be used by people who may find
it difficult to understand or complete the English document.

California law provides individuals the ability to insure that
their health care wishes are known and considered if they become
unable to make these decisions themselves. The California Medical
Association publishes an Advance Health Care Directive Kit which
includes an Advance Health Care Directive form and wallet cards
as well as information which answers the questions commonly asked

information and education resource on funeral planning, financing
funerals, funeral products and services, and grief support and
counseling. You can also post free online obituaries and find
out how to help your child handle grief.

Hospice and Palliative Care Organization - The National Hospice
and Palliative Care Organization (NHPCO) is the largest nonprofit
membership organization representing hospice and palliative care
programs and professionals in the United States. The organization
is committed to improving end of life care and expanding access
to hospice care with the goal of profoundly enhancing quality
of life for people dying in America and their loved ones.

Living Will Registry - The mission of the U.S. Living Will
Registry is to promote the use of advance directives through
educational programs, and to make people's health care choices
available to their caregivers and families whenever and wherever
